Community Care Circle was created with one belief at its core: participants are not passive recipients of care, they are leaders in their own lives.
We are a trauma-informed, culturally responsive NDIS provider committed to building safe, respectful, and empowering support environments. Our work is grounded in co-design, meaning the people we support actively shape how services are delivered.
We listen first. We build together. We act with integrity.
Our approach recognises the impact of trauma, systemic barriers, stigma, and cultural exclusion. We are especially committed to amplifying the voices of people from culturally and linguistically diverse (CALD) backgrounds and individuals whose lived experience has often been overlooked in traditional service models.
Led by experienced professionals in nursing, disability services, governance, and community engagement, we operate in alignment with the NDIS Practice Standards prioritising safety, dignity, transparency, and participant choice at every level.

Our Services
01.Daily Personal Activities
We support participants with everyday personal tasks to maintain independence, health, and wellbeing. Includes: Personal hygiene and grooming Showering, dressing, and personal care Mobility and transfers Mealtime assistance Medication reminders (within scope)
02.Household Tasks
We assist participants in maintaining a safe, clean, and comfortable home environment. Includes: Cleaning and household maintenance Laundry and linen support Meal preparation Organising living spaces Supporting independent living skills
03.Community and Social Participation
We support participants to engage confidently in their communities and reduce isolation. Includes: Attending social and cultural events Support to attend appointments Community engagement activities Building social confidence and independence Cultural and identity-affirming participation
04.Travel and Transport Assistance
We help participants safely access their community, services, and appointments. Includes: Transport to medical appointments Community access support Assistance using public transport Building independent travel skills
05.Assistance with Daily Life in Shared Living
Support for participants living in shared or supported accommodation environments. Includes: Daily routine support Personal care support Promoting independence in shared environments Emotional and wellbeing support
06.Support Coordination
We help participants understand, implement, and maximise their NDIS plans. Includes: Connecting participants with providers Coordinating multiple services Supporting life transitions Advocacy and participant empowerment Crisis and transition support
07.Development of Daily Living and Life Skills
We support participants to build skills needed for independent living. Includes: Communication and confidence building Routine and time management Budgeting and life skills Building independence Emotional resilience support
08.Innovative Community Participation
Individualised supports designed to help participants achieve personal goals and meaningful engagement. Includes: Skill development activities Confidence building programs Mentoring and empowerment support Community integration programs
09.Group and Center Based Activities
Structured programs that promote social inclusion and skill development. Includes: Group wellbeing activities Skill-building workshops Social and recreational programs Peer connection opportunities
10.Accomodation and Tenancy Assistance
We help participants secure and maintain safe and stable housing. Includes: Assistance finding suitable accommodation Support maintaining tenancy Liaising with housing providers Transition support into independent living
11.Assistance to Access Employment or Education
We support participants in pursuing education, training, and employment goals. Includes: Employment readiness support Confidence and skill development Assistance accessing education programs Transition planning
12.Community Nursing Care
Delivered by a Registered Nurse, ensuring safe and professional clinical care. Includes: Medication management (NDIS Module 4.3 compliant) Health monitoring and clinical support Nursing assessments Care planning and coordination Chronic condition support
13.Compliance with NDIS Modules
Community Care Circle operates in accordance with NDIS Practice Standards, including: Module 4.3 – Management of Medication Module 4.5 – Management of Waste Core Module – Participant safety, dignity, and quality care Our clinical governance is led by a Registered Nurse, ensuring safe, ethical, and compliant service delivery.
